7. Please upgrade your USAV membership: GLV/USA INSTRUCTIONS, Go to http://www.greatlakesvolleyball.org, Click on Registration to Register, Please make sure you select Club 1 as your club affiliate. The full membership is $55.00 ($45 if the $10 tryout fee has already been made).
8. Please bring a copy of your daughter’s birth certificate.
9. Please attend the parent drop off tomorrow, Monday, November 4th anytime between 5:30-7:00 p.m. to drop off your completed paperwork, pick up your raffle tickets and try on uniform items. The drop off will be at
Club 1- 14407 Coil Plus Drive, Plainfield.
There will be limited copies of the forms at the open house so please print them if possible.
