2019/2020 STEP BY STEP FOR PROPER REGISTRATION

1. Please watch the informational video at the available link https://youtu.be/fKdfY9anjKI. This video was created to answer questions regarding our program.
2. Please register at the following link to select your program and payment plan. https://club1volleyballreg.sportngin.com/register/form/240091492
3. Please download, print and complete the USAV Medical form and bring to the parent drop off http://www.greatlakesvolleyball.org/Documents/2020%20Player%20Medical%20Release%20w%20ClubTeam%20WITHOUT%20Notary_fillable.pdf

5. Please bring a copy of your son’s birth certificate to the parent drop off.
6. Please attend the parent drop off tomorrow, Wednesday, September 4th, anytime between 5:30 – 7:00 p.m. at Club 1- 14407 Coil Plus Drive, Plainfield to drop off your completed paperwork, uniform try on and pick up raffle tickets.
There will be limited copies of the forms at the open house so please print them if possible.
