7. Please upgrade your USAV membership: GLV/USA INSTRUCTIONS, Go to http://www.greatlakesvolleyball.org, Click on Registration to Register, Please make sure you select Club 1 as your club affiliate. The full membership is $55.00 ($45 if the $10 tryout fee has already been made).
8. Please bring a copy of your daughter's birth certificate.
9. Please attend the parent drop off tomorrow, Monday, October 28th anytime between 5:30-7:00 p.m. to drop off your completed paperwork, pick up your raffle tickets and try on uniform items. The drop off will be at the Community Sportsplex at 24035 Riverwalk Court, Plainfield.
There will be limited copies of the forms at the open house so please print them if possible.
